Applicant has completed, and agrees to abide by, the End Use Statement below.

Submission of this application does not create a dealer relationship until approved in writing by Ballistic Armor Co.

Customer and Vendor End Use Statement

This End Use Statement (this “Statement”) is required by Ballistic Armor Co. to comply with restrictions set forth in U.S. export control laws and regulations. On behalf of Recipient, this Statement is signed by a representative authorized to sign documents on its behalf and to certify this Statement as being accurate and complete.

Recipient acknowledges that he/she/it is receiving the following product(s) from Ballistic Armor:

Recipient understands the Products may be shipped under export license from the U.S. government and are subject to the U.S. Export Administration Regulations and other U.S. laws and regulations;
Recipient further understands that this End Use Statement may be a prerequisite for the issuance of the applicable U.S. export license;
Recipient agrees that the Products shall only be used for the purpose intended as described by Ballistic Armor Co., and by the recipient approved in the applicable U.S. export license;
Recipient agrees to not incorporate the Products into any other products and to not directly transfer, sell, resell, or reexport the Products to any third party located anywhere worldwide subject to U.S. sanctions, nor to allow others to indirectly transfer, sell, resell, or reexport the Products to any third party located anywhere worldwide subject to U.S. sanctions;
Recipient agrees to not transfer, sell, resell, or reexport the Products into another country subject to U.S. sanctions, or otherwise in violation of restrictions in the applicable U.S. export license;
In addition to the above, Recipient agrees to comply with all restrictions regarding the Products described in the applicable U.S. laws and regulations.
